Skin Cancer Risk Assessment

Thank you for completing the risk assessment form.  Your responses have been recorded.

Our doctors will review each completed questionnaire individually and will allocate a risk score.

You will receive an SMS with your risk score, within a week or two.

Your questionnaire will be stored on a medical record in your name at Royal Park Medical.

LOW RISK

Skin check not routine

Your risk of developing skin cancer in the future is low.

You do not need routine skin checks, but should still see a doctor with any concerning (new or changing) skin spots.  If you want a skin check anyway, we can arrange one.  Please contact your employer or reception at Royal Park Medical.

MEDIUM RISK

Skin check not routine

Your risk of developing skin cancer in the future is medium.

You do not need routine skin checks, but should still see a doctor with any concerning (new or changing) skin spots.  If you want a skin check anyway, we can arrange one.  Please contact your employer or reception at Royal Park Medical.

HIGH RISK

Skin check advised

Your risk of developing skin cancer in the future is high.

Your employer will arrange a time for your skin check with our doctors.

VERY HIGH RISK

Skin check advised

Your risk of developing skin cancer in the future is very high.

A regular skin check is advised.  Your employer will arrange a time for your skin check with our doctors.

Skin Check Appointments

If you are LOW or MEDIUM risk, we would not usually recommend a skin check, but we can arrange one if you wish. You will need to contact us to make this request.

If you are given a HIGH or VERY HIGH risk score, you will be invited for a full skin check. This is a very important next step, and will be organised at your place of work and paid for by your employer.

In some cases, our doctors may not be able to make an accurate risk assessment based on the information that you submit (e.g. if you are not confident assessing moles or freckles, or if information is incomplete). If this is the case, we will inform you and invite you to make a longer appointment with one of our doctors to discuss the extra information required and if necessary to perform a skin check.
