If you have any ongoing medical conditions we can transfer some information from your previous doctor, with your permission. The form is available at reception.
Please let us know if you have a “My Health Record”. You can set this up by clicking the link below or via the myGov app. We can access your electronic My Health Record, and can upload health summary information.
