If you have any ongoing medical conditions we will ask to transfer some information from your previous doctor, with your permission. The form is available at reception.
Our clinicians may also access your “My Health Record”. Some people chose to opt out, but if you wish you can set one up in the myGov app. We can upload health summary information to my Health Record.
